Client/Coach Contract
This contract is an agreement entered into by and between: Charlotte Clarke, Charlotte Clarke Coaching and Client Name (to be entered below) whereby the Coach agrees to provide personalised health and fitness coaching as agreed upon by Coach and Client, which involves the following; regular prescribed exercise, nutritional advice and lifestyle improvements for the purpose of eliciting physical fitness goals, improving health markers, enhancing physical performance, developing confidence, strengthening self-esteem or general improvements to quality of life.
Description of Coaching: Coaching is described as an alliance between the Coach and the Client and does not represent a business relationship. It facilitates the personal development of the client through professional expertise of the Coach providing strategy, structure and education to help the Client achieve their agreed upon results.
Purpose of Contact: The purpose of this contract is to ensure it is hereby entered into with full understanding of expectations and agreement of both parties to act in accordance with the requirements of the coaching relationship.
Services Outline: The services to be provided by the Coach to the Client are as follows: a monthly recurring bespoke exercise regiment with the goal to help the Client achieve their desired goals. Furthermore, complimentary nutritional guidance will be provided which comes at the discretion of the Client to adhere to as well as suggested habit and lifestyle factor alterations.
The service will be provided exclusively online via a mobile fitness application, zoom meetings, Email, Instagram and WhatsApp messaging and any other agreed online method.
The service provides monthly exercise routines, with a 3 month minimum commitment, which include full explanations of how each exercise must be performed with included repetitions and sets. Weekly check-ins are provided via a questionnaire and feedback is returned in the form of a written/voice-noted/videoed message.
A monthly 1-1 online call is carried out via voice/phone call or zoom call to review the previous 4 weeks of coaching. It is the responsibility of the Client to book this call with the Coach. At this point any issues and changes may be discussed and updates to the programme will directly follow this call if that is in the best interest of the Client’s progress toward their fitness goals.
Contact with the Coach is of the utmost importance but it is the responsibility of the Client to ensure the Coach is kept properly informed of disruptions to the service by the Client, including but not limited to, missed workouts, lack of adherence to track nutrition, failure to complete progress checks.
The Coach Client relationship continues until the Coach or Client terminates the contract in accordance with the cancellation policy, outlined in this contract.
Expectations: The expectation the Coach has of the Client is to follow the structure, advice and professional support provided. Smart programming is supplied by the Coach but hard work is still required by the Client to ensure that results are achieved. It is expected the Client will give their best effort at every opportunity and inform the Coach when this is not possible.
The Client should not expect to see results if the programme isn’t followed per the request of the Coach and they should expect that true changes to their health to take a considerable amount of time. Therefore, the Client should expect to put in hard work, consistently and patiently, trusting in the processes.
Payment Policy: Payment is collected via recurring monthly payments issued on the programme start date or chosen date specified at the time of sign up. It is the responsibility of the Client to cancel the product in line with the cancellation policy.
Monthly payments can be refunded/paused at the discretion of the Coach when criteria such as extreme unforeseen circumstances arise.
Cancellation Policy: As mentioned in the Payment Policy, cancellation of the service is the responsibility of the Client. The minimum cancellation notice period is one full month i.e. if the Client is mid-way through one month of coaching and wishes to cancel, the remainder of the current month and subsequent month must be completed.
Cancellation of the service can only be invoked by the Client after the 3 month mark. However, the Coach reserves the right to cancel a Client’s service at any time once they have fulfilled the obligations specified in the Service Outline section of this contract.
Refund Policy: Refunds, as mentioned earlier, are provided at the discretion of the Coach. Depending on individual situations a Client may request a refund for the service or to pause their recurring monthly payment, but only after the 3 month minimum commitment period is met. It is the right of the Coach to agree or deny and requests such as refunds or pauses to the programme.

Regular physical activity is fun and healthy, and increasingly more people are starting to become more active every day. Being more active is very safe for most people.
However, some people should check with their doctor before they start becoming much more physically active.If you are planning to become much more physically active than you are now, start by answering the seven questions in the box below.
If you are between the ages of 15 and 69, the PAR-Q will tell you if you should check with your doctor before you start. If you are over 69 years of age, and you are not used to being very active, check with your doctor.
